On November 6, 2021 Kenneth (Kenny) Formby left his earthly home surrounded by his family in Prescott Arkansas. Kenny was born July 16th, 1949 in Houston Texas to Rufus and Edna (Sudsberry) Formby. 

Left to cherish his memory are his wife of 50 years Linda (Ellis) Formby. His daughter Hope and husband Michael Hatley, Son Al and wife Kim Formby of Prescott, his four grandchildren Brooke Formby, Gavin Hatley, Kadence Formby and Payzlee Formby. 

A private memorial service will be held at a later date, in lieu of flowers please donate to the American Cancer Society.
